Please call the number below with billing questions, or contact your insurance company for questions concerning coverage. Physicals: Patients visiting for employment, camp, and sports physicals - please remember to bring your Health Assessment Forms with you. Workers Compensation: If you are injured at work it must be reported immediately to your employer and a written injury report must be filed. Blue Cross & Blue Shield PPO patient claims are processed as a doctor's office visit, in place of an emergency room/urgent care visit. This practice better serves our patients and is due to an enhancement in our contractual agreement with Excellus.
