. Bring a list, or your bottles of current medications. 
 

. Be prepared to tell us about all allergies and surgeries.  
 

. Unless accompanied by a parent or legal guardian,  children under the age of 18 must have written or  documented verbal consent from the child’s custodial parent.

We bill most insurance companies.  Please call the number       below with billing questions, or contact your insurance company for questions concerning coverage.

Physicals:   Patients visiting for employment, camp, and sports physicals - please remember to bring your Health Assessment Forms with you.

Workers Compensation:   If you are injured at work it must    be reported immediately to your employer and a written injury report must be filed.  Please tell us at the time of registration.  

Blue Cross & Blue Shield PPO patient claims are  processed  as a doctor's office visit, in place of an emergency room/urgent care visit.  This practice better serves our patients and is due       to an enhancement in our contractual agreement with Excellus.
